Exploring a Needs Orientated Approach to Care Planning
This assignment will investigate a needs orientated approach to care, critically discussing the nursing process. It aims to show an understanding of what a nursing model and the nursing process is, looking in detail at the relationship between this nursing process and the Roper, Logan and Tierney (RLT) model of nursing. This essay will explore how the nursing model and process is implemented in practice, considering how the RLT model assists nurses to adapt a problem-solving technique when developing care plans for individuals. The first stage of the process is assessment. Roper et al (2001) refer to this process as ‘assessing’ indicating an ongoing activity; this encourages nurses to recognise the on-going nature of this initial phase. The assessing stage includes gathering information about a patient, reviewing this information, identifying actual and potential problems and prioritising (Roper et al 2001). Roper et al (2001) explain the importance for assessing, as early as possible in the patient’s stay. Extensive, in-depth information may not be gathered on an initial assessment, however any information obtained contributes towards individualised care (Roper et al 2001). Ambrose and Wittig (1998) explain that the initial assessment becomes a foundation for ongoing assessing and holistic care. Barrett, Wilson and Woollands (2009) concord with Roper et al and Wittig in that assessing is an ongoing process and elaborate on this explaining that assessment should not be confused with admission. They state “an admission tends to be a one-off process when you first meet the patient, whereas assessment carries on throughout your relationship with the patient” (pg22). Assessment enables the nurse and patient to identify actual and potential problems. Although, some problems can be directly related to biological needs, holistic needs must be considered, i.e. psychological state and cultural/social standing
Over the past 30 years nursing has evolved from a task-oriented to a logical and systematic approach to care, using theories and models to guide practice (Pearson et al, 1996). Being a nurse is not as easier as simply providing nursing care according to individuals’ disease during hospitalization. Neuman (1980) describes the person as a whole system and recognizes that there are common features in any species and a core of such features in people.Also, nursing model is not restricted to nursing but can be shared by anyone of health professional. (Neuman, 1995) .Therefore, nursing models have been variously referred to as philosophies, conceptual frameworks, paradigms, theories and metatheories (Fawcett, 1995) that can give some guide to nurses working in a particular area.
One of the widely used nursing model is Roper-Logan-Tierney model of nursing which raised out by Nancy Roper and two of her colleagues (2001) emphasizes the importance of the patient’s ability to perform activities of daily living. It vies individuals as being engaged in various activities of living throughout their lifespan where will fluctuate between total independence and total dependence to age, circumstance, and health status. It is referred as a human need model. In cardiac ward, Roper-Logan-Tierney model is one of the theory that is utilized in the health care setting. Nurses apply this model because from here they can assess the activities of daily living of patient so that to provide a comprehensive picture of an heart disease patient’s lifestyle especially with the way patient manage their diet restrictions, medications regimen, and choice of treatment or potential problems which may require the focus of nursing care.
This assignment may identify and how the theory can be applied in nursing career by some examples. And emphasize the strengths, limitations of Roper-Logan-Tierney Theory and state its value and relevance to the current nursing practice.
Roper-Logan-Tierney Model of Nursing
The Roper-Logan-Tierney model first developed in 1980, this is currently used today only as a checklist on admission rather than an approach to assessment and ongoing care of an individual. It is often used as a way of for nursing was base, from the outset, on the model of living.
Nursing Practice with Roper-Logan-Tierney model
Acute coronary syndrome (ACS) is a leading acute health problem throughout the world. Upon admission, patients are being assessed using the institution’s assessment tool. There’s a portion on that tool that assesses patient’s level of independence in performing different activities of living like, eating, communicating, mobilizing, eliminating, hygiene and etc. This will serve as a baseline, preoperatively, to determine the extent of impact of heart attack on the patient’s independence, postoperatively, considering the factors mentioned by the said theory and consequently the basis for providing nursing care. This determines the activities of living patients need most assistance.
Roper Logan Tierney’s Theory is one of the theory that is utilized in the health care setting. The Leininger’s cultural care diversity and universality theory provides a broad, holistic and comprehensive view of human beings, both individually and as a group. It is a helpful guide for studying cultures, implementing client care in a number of settings and performing qualitative research to discover care and health in diverse cultures. Also with the application of culturally congruent care, nursing care can best fit with a client’s culture. Then culture stress is decreased and conflicts between client and nurse can be avoided.
